Trauma-informed treatment and support services

to homeless adults living with a major mental illness, who may also have a substance use disorder. To achieve recovery, these individuals require a safe, supportive residence while permanent housing and linkage to community resources are established.

The Bridge is a 12-bed 24-hour residential treatment program that uses an integrated health model, recognizing the unique needs of each resident. We seek creative and individualized ways to offer life-affirming and empowering experiences, and work to provide an environment where individuals can maintain individuality and dignity, access to community opportunities, and enhance their independence and quality of life. We provide trauma-informed treatment and support services to adults living with mental illness and substance use disorders. 

 

PROGRAM SERVICES AND STRATEGIES INCLUDE: 

  • Assistance with activities of daily living 

  • Medical monitoring and medication administration  

  • Individualized mental health treatment (individual and group therapy) 

  • Transportation to recreational activities and appointments 

  • Coordination and/or referrals to community resources (e.g. primary care, mental health and substance use services, peer recovery programs, housing, support groups, financial support, educational services, and employment resources.) 

  • Crisis management services 

  • Organized community outings 

  • Trauma informed care  

  • Meal preparation 

 

POPULATION SERVED 

Individuals, 18 years of age or older, who have been diagnosed with a major mental illness and may or may not have a substance use disorder.
