The Project for Assistance

in Transition from Homelessness (PATH) team provides outreach, engagement, and referral services for people with mental illness and those with co-occurring substance use disorders who are experiencing homelessness in York and Cumberland County.

 

Our PATH program provides outreach to homeless individuals (sleeping outside, in a vehicle, or any place not meant for human habitation) struggling with mental health or substance use, and currently unconnected to resources. The PATH program team provides short-term intensive outreach, engagement, and case management services and connects clients to resources for food, housing, social security, medical care, substance use treatment, and longer-term mental health services.
