Crisis Aftercare Program
What services does this program offer?
The program offers a 45-day episode of crisis services to children who are experiencing a crisis. An assessment, safety planning, and follow-up will occur. A specialized crisis worker (Child and Family Support Specialist) can go into the home as much as is useful to the family during that 45-day period and offer support. This support consists of skill-building, coordination of existing services, referral to new services, and other interventions aimed at stabilizing the crisis and providing the family with the resources needed. This is especially intended to keep children in their home and community instead of needing to access higher levels of care – such as hospital.
Who is eligible for the service?
Individuals who are in Cumberland County - age 0-20 years old who are experiencing a mental health crisis and could be helped by additional support from the Crisis Aftercare team.
